If you have never sent UBs before, you must obtain a UB-04 billing manual from your local Hospital Association. This provides you with the standard information needed on the UB-04 for your area. If you will be printing paper UB-04 claim forms, Dot Matrix printers are preferable for alignment, but they can also be printed on a laser printer.
Perform the steps contained in the UB-04 Setup and Troubleshooting document.
After all necessary data is set up and charges have been entered you can
proceed with the normal Insurance
