Insurance Claim Form Wizard

With the Insurance Claim Form Wizard function, you can control the alignment and positioning of the data that is printed on the insurance claim form for each data field on the CMS 1500 Form (previously HCFA).

The Insurance Claim Form Wizard provides the following functions:
Upon accessing this function, any previously stored profiles will be listed. You can edit, print, delete, or review a profile, by selecting it. To select a profile, click anywhere in the row containing the profile name.

Add a Printer Profile
The alignment settings are stored individually for each printer. If you use multiple printers for printing insurance forms, you may need to add multiple profiles. To add a new printer profile, click Add in the Action Column. The printer profiles only apply to Windows printers or the 'Print Here' printer function - they cannot be setup for Legacy printers.

NOTE: If your practice uses multiple databases in CGM webPRACTICE, the profiles are not database specific. Any changes made to a profile in one database will be made to that same profile across all databases. If you need different alignment settings by database, you need to create a different printer profile in each database and ensure that each database's users know which profile to select for printing. After creating the printer profile, the insurance claim form wizard will display. You must select a field first before any alignment changes can be made. You can use the functions in the Action Column to select portions of the form or you can select an individual field by clicking on it. If you want to select multiple fields, hold down the CTRL key while clicking each field. After you select the field(s) use the arrow keys on the keyboard to change the alignment settings. Each time you press an arrow key, the field(s) will be moved 1/16th of an inch. Pressing an arrow key twice will move the field approximately the space of one character.

Test Print
Click Test Print in the Action Column to print a test form for this printer profile.

Reset
Click Reset in the Action Column to reset the alignment back to the original default settings.

Select Select Top, Middle, or Bottom
You can quickly adjust an entire section of the insurance form using these functions. Click the applicable function in the Action Column and then use the arrow keys to change the settings.

Select Entire Form
You can adjust every field on the insurance form by selecting the entire form and using the arrow keys to change the settings. Click any area on the form that does not contain a field to select the entire form. For example, at the top of the form you can click the blank area to the left of the insurance company name and address field. The outside of the form should be outlined with a red box after you select the entire form.

Clear Selections
Click Clear Selections in the Action Column to clear the current field selection(s).
