The Line Item Note Attachment is used to capture free text notes that are specific to a particular procedure code. When an insurance claim is printed, the descriptions from any NOTE attachments print above the corresponding procedure code line item in Box 24a on the CMS-1500 form. A 'ZZ' qualifier prints in front of the note.
