CLIA Number Setup Process

If your practice needs to send a Clinical Laboratory Improvement Amendments (CLIA) number on claims (at the claim level-Loop 2300), you need to perform the following three steps. If you need to send a CLIA number at the service line item level in Loop 2400 (REF with F4 qualifier), you need to use the Outside Lab Attachment when posting procedures.

Step 1 - Enter CLIA Number in Location Code Table
In Maintain Location Codes, enter the CLIA number in the CLIA Number field for each location that you bill CLIA-required lab services from. It is possible to have different CLIA numbers based on location.



Step 2 - Indicate CLIA Number for Specific Types of Service
In Maintain Type of Service Codes, select the Include CLIA Number on Claims check box for all types of service that require a CLIA Number.



Step 3 - Verify Information in Billing Profile Rules
In Billing Profile Rules, ensure that the correct CLIA Number option is selected in your Default Profile and any additional profiles you might have set up.
Use Location CLIA Number: Select this option to use the CLIA Number stored in Maintain Location Codes for the location selected on the charges.
Use: Select this option and type a CLIA Number to use on all claims whether or not a CLIA Number is stored in the Location Code Table.
